The Aesthetic Plastic Surgery Center has created this privacy statement in order to demonstrate our firm commitment to privacy.The following discloses our information gathering and dissemination practices for this website. Your Information
We respect your privacy. Any information we collect from you, including your email address, will simply be used to send you the information you have requested. We will not give or sell your personal information to any unaffiliated third party. If you give us your email and permission to send more information, you may opt out of receiving any additional email messages at any time, by unsubscribing from our email list.We use your IP address to help diagnose problems with our server and to administer our website by (1)identifying which parts of our site are most heavily used (2) to tailor site content to user needs and (3) togenerate aggregate statistical reports. We do not link IP addresses to anything personally identifiable. This means that user sessions will be tracked, but the users will remain anonymous.You need not provide the Surgery Center with personally identifying information (e.g., name, telephone number, email address, city and/or state of residence) as a condition of using our site. We use a tool called “Google Analytics” to collect information about use of this site. We use the information we get from Google Analytics only to improve our website. Google Analytics collects only the IP address assigned to your browser on the date of your visit, instead of your name or other identifying information. We do not combine the information collected through the use of Google Analytics with any personally identifiable information. Although Google Analytics plants a “cookie” on your web browser to identify you as a unique user the cookie cannot be used by anyone but Google. Google’s ability to use and share information collected by Google Analytics about your visits to this site is restricted by the Google Analytics Terms of Use and the Google Privacy Policy. Our website offers the ability to subscribe to newsletters that may offer services, promotions, and special event invitations. You can opt-out of these newsletters at any time by selecting the “unsubscribe from this list” option at the bottom of the e-mail.

The Aesthetic Plastic Surgery Center is committed to protecting the privacy of all of our patients’ personal health information. Part of that commitment is complying with the Privacy Rule of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), which requires us to take additional measures to protect personal information and to inform our members about those measures. Our notice of privacy practices provides information about how we may use and disclose protected health information about you.
The notice contains a patient rights section describing your rights under the law. You have the right to review our Notice before signing this consent. The terms of our Notice may change. If we change our notice, you may obtain a revised copy by contacting our office. You have the right to request that we restrict how protected health information about you is used or disclosed for treatment, payment, or health care operations. We are not required to agree to this restriction, but if we do, we shall honor that agreement.
The patient understands that: Protected health information may be disclosed or used for treatment, payment, or health care operations. The practice has a Notice of Privacy Practices and that the patient has the opportunity to review this Notice. The Practice reserves the right to change the Notice of Privacy Practices. The patient has the right to restrict the uses of their information but the practice does not have the right to agree to those restrictions. The patient may revoke this consent in writing at any time and all future disclosures will then cease. The Practice may condition receipt of treatment upon the execution of this consent.
